Problem Report Form

Use this form to report any type of electrical problem that our crews might need to look into. The form will be emailed directly to our Electric Distribution Department where the problem can be dealt with. When filling out the form, pay special attention to giving the pole number nearest to the problem area. The pole number is located about 5 ft above the ground and is usually facing the street. If you can't find the number, just give the nearest address.
